8VAC20-780-40-M · The center shall maintain, in a way that is accessible to all staff who work with children, a current written list of all children's allergies, sensitivities, and dietary restrictions documented in the allergy plan. This list shall be dated and kept confidential in each room or area where children are present. Child #3 has milk sensitivity, Child #5 has a nut allergy and Child #6 has an egg allergy and their names were not written on a current list in their classrooms. These 3 children are in sperate classrooms.

Evidence: In the Infant room, there was observation of a 2:10 staff to children ratio during the inspection. Interview with staff confirmed the staff to children ratio followed.

8VAC20-780-410-1 · Based on observation of center's video footage, interview, and review of documentation, the center did not ensure all staff refrained from forbidden actions.

Evidence: On August 19, 2022, there was a review of video footage capturing the incident on August 18, 2022 involving Staff#1 interactions with Child#1 and Child#2 in the classroom. The observations consists of the following: 1. Child#1 and Child#2 were separated and placed on opposite sides of the green wall. 2. Staff#1 roughly grabbed Child#1 by the arm and directed child to sit down on the floor. The second time Staff#1 grabbed child by the arm while Child#1 was standing and roughly sat child back down on the floor . The third time Child#1 was sitting down and Staff#1 grabbed child by the arm and roughly sat the child down on the floor. 3. Child#2 attempted to go to the other side and Staff#2 immediately grabbed child by the hand, roughly and directed child to sit down. 4. Interview with staff confirmed observations in the video footage. 5. Documentation reviewed confirmed incident observed at the center.

Evidence: 1. In Staff#2's record (date of hire: 01/17/22), there was not a copy of a sex offender registry check found. 2. Interview with staff confirmed a copy of this search was not completed prior to this inspection, however a search has been completed and included in staff's record at this time.
