22VAC40-185-340-F · Based on interviews and documentation review, the licensee did not ensure all children under 10 years of age shall always be within actual sight and sound supervision of staff.
Evidence: Staff #1 confirmed during a fire evacuation drill on July 19, 2021, Child #1 was left in the classroom without supervision of center staff. Center staff evacuated 8 children from the Toddler room during a fire drill. Once staff and children were outside, Staff #2 reviewed center documentation (digital child roll list), conducted a head count and realized Child # 1 was missing. Staff #3 went inside to the classroom and retrieved Child #1 who was left sleeping in a crib. Child #1 was left unsupervised for approximately one minute.
