Evidence: Staff #1 confirmed during a fire evacuation drill on July 19, 2021, Child #1 was left in the classroom without supervision of center staff. Center staff evacuated 8 children from the Toddler room during a fire drill. Once staff and children were outside, Staff #2 reviewed center documentation (digital child roll list), conducted a head count and realized Child # 1 was missing. Staff #3 went inside to the classroom and retrieved Child #1 who was left sleeping in a crib. Child #1 was left unsupervised for approximately one minute.

22VAC40-665-540-A · Based on record review and staff interviews, the licensee did not ensure staff shall be evaluated by a health professional and be issued a statement the individual is determined to be free of communicable tuberculosis (TB). Documentation of the screening shall be submitted at the time of employment and prior to coming into contact with children. The documentation shall have been completed within the last 30 calendar days of the date of employment and be signed by a physician, physician's designee, or an official of the local health department.

Evidence: The following staff did not have a TB screening conducted prior to employment. 1) The TB screening for staff #2 was dated 10-1-2021 and date of employment was 9-27-2021. 2) The TB screening for staff #3 was dated 10-1-2021 and date of employment was 9-21-2021. Documentation was confirmed by center director.
