Child #1 was missing the home address for both emergency contacts listed. Child #3 was missing one parents employment location and phone number and the complete address for one emergency contact listed.

employment or 90 days from subsidy vendor approval a current certification in cardiopulmonary resuscitation (CPR) appropriate to the ages of children in care. The training shall include an in-person competency demonstration. Staff #5 was been working in the center working for more than 26 years and does not have CPR certification.

These children have been enrolled for more than 5 weeks.
