Two containers of disinfectant wipes with the warning labels, "Keep out of reach of children," were unlocked in the four and five year old and the two year old classroom, within reach of children. A container of disinfectant wipes with the warning labels, "Keep out of reach of children," were unlocked in on top of the diapering station in the girls bathroom.

8VAC20-780-350-B-4 · Based on observation and staff interview the licensee did not ensure to maintain required staff-to-children ratios whenever children are in care.

Evidence: The required ratio for 3 year old children is 1 staff for every 10 children. The following classrooms were out of ratio at the time of the inspection. 1. Room #108 there were 11 children present with 1 staff member. 2. Room #110 there were 11 children present with 1 staff member.

8VAC20-780-80-B-2 · Based on record review and staff interview the center did not inform the department?s representative as soon as practicable, but not to exceed two business days, of any injury to a child that occurs while the child is under the supervision of the center and requires outside medical attention.

Evidence: On December 2, 2022 Child #1 (age 3) sustained an injury which required outside medical attention. The incident was not reported to the department.

Evidence: Five staff Records were reviewed. The record for staff #4 who began employment at the center 5/11/2021 contained a signed Sworn disclosure or affirmation statement which indicated that staff #4 lived in North Carolina within 5 years of employment. The record for staff #4 did not contain the results of a criminal history record information check and a search of the child abuse and neglect registry or equivalent registry from the state of North Carolina. The program director confirmed that this documentation had not been obtained for staff #4.
