Classroom #1 places infant pacifiers into the sanitizing solution that is located in the kitchen for the use of sanitizing the dishes. The sanitizing solution is labeled "keep out of reach of children" and "danger." The solution also states "harmful if swallowed." After placing the infant pacifiers into the sanitizing solution, classroom #1 rinses the pacifiers off and gives the pacifiers to unassigned infants.

Evidence: The refrigerator in classroom #1 contained 2 bottles with prepared infant formula that were not dated or labeled with the child's name. Staff in classroom #1 confirmed that the 2 bottles were not dated or labeled with the child's name.

Evidence: 1. The KCE Brandermill Diapering policy training states that "every child in our care, regardless of age, that wears diapers or pull-ups will be changed every 2 (two) hours or as soon as the teacher notices that they have soiled the diaper regardless of wet or bowel movement to ensure proper hygiene." 2. The KinderCare app indicated that on 06/05/2024 child #1 had a diaper change at 10:34 am. The app did not notate any other diaper changes occurring on 06/05/2024. 3. Interview and written statement from staff #1 determined that child #1 did not have any other diaper changes on 06/05/2024 after the 10:34 am diaper change. 4. Child #1 was picked up from the center at approximately 3:37 pm.

Interview with staff #2 confirmed that they lived in a state outside of Virginia in the preceding 5 years. Administration confirmed that the criminal background check was not requested prior to staff #2 beginning employment and a central registry background check was not requested no later than 30 days after beginning employment from that state.

Evidence: 1. The record for staff #1, date of employment 09/23/23, indicated that staff #1 lived in another state in the preceding five years. 2. Interview with administration revealed that the center did not obtain a search of the child abuse and neglect registry from that state by the end of the 30th day of employment.
