    8VAC20-780-350-C · Based on information obtained during the inspection, the center did not ensure staff to children ratio requirements applicable to the youngest child in the group were followed when children were regularly in mixed age groups.

    Evidence: In class #4, a review of the staff to child ratio was conducted using the facility?s Face-To- Name Transition sheet for 9/01/22. The following ratios and times were recorded on the Face-To-Name Transition sheet by staff #1. At 6:55 AM, 1:11 ratio with the youngest child two years of age. At 7:10 AM, 1:11 ratio with the youngest child two years of age. At 7:15 AM, eight pre-k children are separated from the group to go to class with staff #2. The Face-To-Name Transition sheet continues to document the following times and ratios: 7:53 AM 1:11 ratio; 7:55 AM 1:12; 8:00 AM 1:14 with the youngest child two years of age. At 8:05 AM, five preschool children are separated from the group to go to class with staff #3 At 8:25 AM, staff #4 arrives leaving the ratio 2:10 with the youngest child age two years. At 8:47 AM, 2:14 with the youngest child a one year old. At 8:55 AM, 2:15 with the youngest child a one year old. At 9:06 AM, 2:16 with the youngest child a one year old. At 9:08 AM, 2:17 with youngest child one year old. At 9:10 AM, 2:19 with the youngest child one year old. At 9:15 AM, staff #5 arrives and three children are transitioned to another classroom leaving the ratio at 2:16 with the youngest child age two years. When children are regularly in ongoing mixed age groups, the staff-to-children ratio applicable to the youngest child in the group shall apply to the entire group. The ratio for children 16 months to 24 months is 1:5 and the ratio for children two years old is 1:8.
