    8VAC20-780-340-A · When staff are supervising children, they shall always ensure their care, protection, and guidance.

    Staff reported during interviews on September 10th, 11th and 16th, 2025, that they observed Staff #1, eating the children?s personal food in Classroom # 1, during feeding times. Staff #1 was observed drinking the juice from the children?s fruit cups and eating fruit from their utensils during breakfast time. Staff also reported during interviews on September 10th, 11th and 16th, 2025, that teachers were observed in Classroom #1 kissing the children on their faces. Upon reviewing the Bright Wheel application used at the center, to log infant feedings, diaper changes and nap times; documentation for diaper changes were not consistent to the center?s policy of performing diaper changes every two hours. There were seven children enrolled on 9/16/2025, the Licensing Inspector reviewed diaper changes from 9/1/2025- 9/15/2025. Six out of the seven children were not consistently changed every two hours, at least one day between the dates of 9/1/2025-9/15/2025. Child # 1 was not changed for four hours on 9/6/2025. Child #2 was not changed for three hours on 9/5/2025, and not changed for four hours on 9/6/25. Child # 3 was not changed for three hours on 9/4/2025, and not changed for three hours on 9/9/25. Child #5 was not changed for five hours on 9/6/25, and not changed for three hours on 9/9/25. Child # 6 was not changed for three hours on 9/5/25, and not changed for four hours on 9/10/25. Child #7 was not changed for 3 hours on 9/7/25, and not changed for three hours on 9/13/25.

    Based on interview and documentation reviewed, a child with a diagnosed food allergy was served a food identified in the child?s written allergy care plan. Evidence: On 07/31/2024 the parent of C1, age 5 had a food delivery service deliver a McDonald?s meal to the daycare center for C1?s lunch. C1 picked up one of the two delivery bags that were taken to the classroom from the delivery service and consumed a chocolate milk that was in the bag. Staff, S1 observed C1 drinking the milk and stopped her. Center staff report that C1 drank approximately 1/2 of the milk in the carton before the staff could intervene. The allergy care plan for C1 contains documentation of diagnosed allergies to dairy.
