    The center must establish and implement procedures for the appropriate supervision of children in care. In or around September of 2025, Child #1 was inappropriately touched by another child while unsupervised in the preschool bathroom. On a separate occasion, witnesses stated that unsupervised children had been observed playing in the bathroom, and teaching staff were notified. The center administration stated that their policies included staff being stationed in locations that enabled them to be able to effectively supervise and keep the children safe.

    22.1-289.031-B-6 · Based on record review and interview, the center did not establish and implement a procedure to ensure that all staff are able to recognize the signs of child abuse and neglect.

    Evidence: 1) Six staff records were reviewed during the inspection. The records did not contain documentation that staff have received training to be able to recognize the signs of child abuse and neglect. 2) During interview, management stated staff have received information on recognizing the signs of child abuse and neglect, but the center did not require staff to complete child abuse and neglect training prior to employment and working with children.
