8VAC20-780-340-A · When staff are supervising children, they shall always ensure their care and protection.
(Repeat Violation and Systemic Deficiency) On 2/23/2026, Staff #1 forcefully pulled Child #1, (age two), by their right wrist and moved them approximately six feet across the floor until the child stood up on both feet. Child #1 was visibly upset and reportedly cried. On the same day, Staff #1 lifted Child #2 (age two) by the upper arms and forcibly placed the child at their eye level, in close proximity to their face.

On March 23, 2026, Staff #1 forcefully walked Child #1, age 3, approximately three feet while holding the child?s hand above their head. Staff #1 pushed the child forward while releasing their hand and in an elevated tone, instructed them to sit on the border that surrounded the play equipment. Child #1 cried and was consoled by a witnessing staff member.

In the center's handbook the supervision policy states, ?that children under 10 years of age always shall be within actual sight and sound supervision of staff?. The center?s policy also states that ?taking roll and keeping an accurate roll book are vital to the safety of the children in care. Staff shall always be able to account for the children in their care, and the office staff shall immediately be notified of lost or missing children?. On December 5, 2025, Staff #1 was gathering a two-year-old class at the main entrance of the school age building. Staff #1 was in the process of transitioning the children back to the preschool building. Staff #1 did not ensure that all children in care were accounted for prior to leaving the building. During the transition, Child #1 climbed into a "buggy? that was being stored in the hallway next to the door. Staff #1 was not aware that Child #1 had climbed into the "buggy" and was left in the building. Child #1 climbed out of the buggy and walked down the hall towards the back of the building and entered into an empty classroom where the child remained for less than 1 minute. When Child #1 entered back into the main hall, a parent saw the child and walked the child down to Staff #2?s classroom. Staff #2 then called admin to inform them that Child #1 was left alone in the building. Admin came to retrieve Child #1 and transitioned the child back to the preschool building. Child #1 was left unattended for approximately 4 minutes and showed no signs of distress.

On October 24, 2025, Staff #1 gathered five preschool age children to transition from the preschool building to the school age building. During the transition, Child #1 (3-year-old) was seen walking along with Staff #1 and the five preschool age children. Child #1 fell on the ground and Staff #1 and the 5 preschoolers continued walking to the school age building. Child #1 was left outside alone for approximately 2 minutes before Staff #2 retrieved the child and brought Child #1 inside.

The infant?s daily schedule shows that the child has a bottle every 2-3 ? hours.
