Evidence: in the infant B classroom, two children were listed on the attendance record, however four children were observed in care; the preschool 3 classroom was observed on the playground without an attendance record for 9/9/24, and five children were observed in care; in the prepper?s classroom, observed during a fire drill, eight children were listed on the attendance record, however 10 children were observed in care.

8VAC20-780-400-B · Based on multiple interviews and a review of documentation, the facility failed to ensure that behavioral guidance was constructive in nature, age and stage appropriate and intended to redirect children to appropriate behavior. Evidence: On 8/18/2023, at approximately 3:00pm, staff #1 put their hand on the face of child #1 (age six) ?smooshing it.? Interviews revealed that child #1 had been attempting to go inside a classroom to say hello to their sibling. Sometime between approximately 4:00-5:00pm, child #1 was observed hiding in a corner crying. Staff #2 asked child #1 what was wrong, and child #1 stated, "the teacher pushed me in the face" and "my head went back." Child #1 then demonstrated a claw like hand pushing their head back.
