On 12/17/2024 at around 4:30 pm in the toddler classroom, Staff A observed child A hit a classmate in the face. Staff A then reportedly smacked child A on the left side of the face. Child A reportedly cried for about one minute after the incident.

8VAC20-780-40-E · Based on review of the center's policies and procedures, and staff interviews, it was determined that the center failed to ensure that staff followed the center's own policies and procedures required by the standards for identifying where attending children are at all times. Evidence: 1) The center?s policy "5-26 Child Safety" states: "Never leave a child unattended. Always count your children. Be sure to count them before and after transitions, such as going to the bathroom, coming in from the playground?." 2) On the incident date of 3/5/2024, between approximately 10:45-11:15 am, child #1, age two, was left unattended in the enrichment room for approximately 2-5 minutes. 3) Staff stated that they did not count the children before and after leaving the enrichment room or before and after arriving in the two year old classroom, as required.

Evidence: The center is licensed as The Compass School of Manassas, LLC and is doing business as The Gardner School of Manassas. Enrollment packets are labeled "The Gardner School", signage within the building has been changed as have several forms used within the center.

8VAC20-780-340-A · Based on observation, it was determined that staff did not always ensure children's care, protection and guidance.

Evidence: When the Licensing Inspector (LI) and director entered the preschool playground, a child was observed sitting alone, by the classroom door, visibly crying and told the director and LI ?I fall down, ice?. While on the playground, the LI observed two children intentionally walking and slipping on a large patch of ice on the sloped grass area on the playground. Staff were observed on the playground with the other children and it was not until the director told staff to keep the area off limits did staff intervene.

Evidence: Child #5 has a diagnosed food allergy to eggs, Child #6 has a diagnosed food allergy to peanuts and Child #7 has a diagnosed food allergy to peanut, tree nuts and there was no allergy care plan in the file.
