8VAC20-780-40-E · The operational responsibilities of the licensee shall include ensuring that the center's activities, services, and facilities are maintained in compliance with these standards, the center's own policies and procedures that are required by these standards, and the terms of the current license issued by the department.

It was determined through staff interviews that staff did not comply with the facility's policy of conducting "Name to Face Recognition Checks" during every period of transition. On February 25, 2026, a final observation of the classroom was not conducted after the name to face recognition check was completed and before transitioning out of the classroom which resulted in a two year old child being left alone in the classroom. A " Name to Face Recognition Check" was not completed at the second doorway before the children were allowed to exit building to the outdoor play area. This resulted in staff not being able to confirm whether all the children were present and accounted for.

It was determined through staff interviews that infant # 1, age 10 months and able to crawl, was placed approximately 4 feet away from the door to the playground which was being held open by a staff #1 being handed another infant by staff #2 to be brought into the classroom. Staff #1 did not recheck the surrounding area before allowing the door to shut. Infant #1 had crawled towards the door, and their finger was caught in the doorway which caused a deep laceration on the infant?s right ring finger.
