The Toddler classroom had chipped and cracked wood on the back wall by the window. The Two's classroom had chipped and cracked wood on the far right-side wall next to the cots. The Four's classroom had chipped and cracked wood on the left wall next to the Dramatic Play area. The School Age classroom had a missing wooden panel on the preparation table next to the main classroom door. The preparation table also had an area of cracked wood on the door.

Evidence: There was approximately 3 to 4 inches of resilient surfacing (mulch) in the fall zones on the playground. There should be 6 inches of resilient surfacing (mulch) in the fall zones.

8VAC20-780-340-A · Based on staff interviews and documentation, facility staff failed to ensure the care, protection, and guidance of all the children enrolled in the program. Evidence: Staff #1 unhooked child A?s safety straps of the highchair before removing the tray. Due to Child A being unsecured in the seat when the tray was removed, he fell out of the highchair and forward on to the floor. The highchair?s seat height was 9 inches from the floor. Child A sustained a large bump on the front left side of his forehead that appeared immediately after the fall. It was confirmed through staff interviews that training was received regarding the appropriate procedure to remove a child from a highchair.
