22VAC40-185-410-1 · Based on review of video recording from provided by the facility and interviews conducted, a staff used the forbidden action of rough handling of a child to correct the behavior of the child. Evidence: On May 25, 2021 staff A roughly handled child 1 to redirect their behavior by grabbing the child by the arm and pulling them to sit by the wall for a time out. The child was pulled by the arm a second time and slid back up against the wall after they scooted on the floor away from the area which they were originally placed for time out.

Evidence: The center's policy states "Name to Face should be done each time you move the children as a group or separately." On 7/10/2024 at approximately 10:30am, Staff #1 and Staff #2 did not complete Name to Face procedures when their class returned back inside the building following outdoor time, resulting in Child #1 (Age 21 months) being left alone on the playground.

Evidence: In the Infant room, there were two bottles in the refrigerator that were not dated and one bottle with no name or date. In addition, there was two bags of animal crackers and a puree container on the counter that had no name or date.
