On 4/8/26 when Child #1 (2 years old) left the preschool playground, Child #1 proceeded through a latched metal gate on the preschool playground and then an unlocked wooden gate surrounding the field. At the other end of the field, a gate exits to a parking lot. Staff reported that Child #1 has been trying to exit the playground from these gates for approximately 1 month, that staff have reported the gate lock is broken, and that there has been no resolution to keep an incident from occurring.

Evidence: Upon interview with staff at the center, it was reported that Child #1 was expressing challenging behaviors including throwing themself on the floor in a tantrum, throwing items in the classroom and screaming loudly. Staff #1 became frustrated with the child and pinched them on their arm as a form of punishment.
