On 4/8/26 when Child #1 (2 years old) left the preschool playground, Child #1 proceeded through a latched metal gate on the preschool playground and then an unlocked wooden gate surrounding the field. At the other end of the field, a gate exits to a parking lot. Staff reported that Child #1 has been trying to exit the playground from these gates for approximately 1 month, that staff have reported the gate lock is broken, and that there has been no resolution to keep an incident from occurring.

Evidence: 1)During inspection in the Sunshine and Caterpillar classroom restroom area (ages 3 months-2 years old), there was a can of disinfectant spray stored inside the unlocked adult restroom stall. 2)During inspection in the Sunflowers classroom (ages 20 months ? 2years old) there was a tube of toothpaste stored on the countertop. 3)During inspection in the Sunflowers and Butterfly classroom restroom area (ages 20 months-2 years old), there was a can of disinfectant spray and a can of air freshener stored inside the unlocked adult restroom stall.

Evidence: 1)During inspection in the laundry area directly off the main hall where there is no door or lock, a jug of clothes detergent was stored on the floor. 2)During inspection in the resource area directly off the main hall where there is no door or lock, there were 26 containers of disinfectant stored on an open shelf.

8VAC20-780-410-1 · Based on staff interviews, the center did not ensure that the forbidden action of physical punishment of a child did not occur.

Evidence: Upon interview with staff at the center, it was reported that Child #1 was expressing challenging behaviors including throwing themself on the floor in a tantrum, throwing items in the classroom and screaming loudly. Staff #1 became frustrated with the child and pinched them on their arm as a form of punishment.

Evidence: 1)In the kitchen area off of the main hallway there is no door and the cabinet under the sink was unlocked and contained 2 gallons of bleach. 2)In the teacher resource area off of the main hallway there is no door and there was a box of bleach sitting in the entryway to this area. 3)In the janitorial closet off the main hallway, the door was unlocked and there was a multi-surface cleaner, bleach and glass cleaner located inside the closet.
