Evidence: Interviews revealed that on 2/7/19 at approximately 8 am, child A, 2 years of age was playing and fell into the glass door of the gas fireplace. The gas fireplace was on and the glass door was hot and caused a first degree burn on the child's hand.

Evidence: 1. In the basement, which is the main programming space for child care, there was an unlocked refrigerator accessible to the children in care. On the second shelf from the bottom on the door of the refrigerator, there was a bottle of Amoxicillin and Children's Tylenol. On the third shelf of the door of the refrigerator, there was a bottle of Cetirizine and cough syrup. 2. In the basement, in the bathroom that was designated for child use, on the top shelf of the bathroom cabinet, there was a box of unlocked heartburn medication.

Evidence: Upon arrival, the Licensing Inspector observed 17 children with one caregiver. Interview revealed that 3 of the 17 children resides in the home, therefore the remaining 14 children were enrolled in the family day home. The family day home has a licensed capacity of 12 children.
