Evidence: Staff of the center who are all mandated reporters did not report suspected child abuse and neglect immediately as required. Interviews revealed that staff reported concerns of abuse and neglect to management. On March 27, 2024, an incident occurred in which staff witnessed another staff roughly grab and hit a child. As of April 3, 2024, the incident had not been reported.
