There were three children present, and the staff member reported that the third child had arrived at approximately 9:30am. 2. On 10/18/23, at approximately 11:53am, the attendance document in the ?Frogs? class for two-year-olds contained documentation of five children present. There were four children present, and the staff member reported that the fifth child was transitioning and moved back to the Toddler class at approximately 11:40am for nap.

Evidence: On 9/6/22 the ?Ducks'' class (two-, three-, and four-year-olds) were observed playing on a yellow school bus climber on the playground. There was approximately two inches of rubber mulch under/around the climber, with grass showing through the mulch. The requirement for a sufficient fall zone is at least six inches of rubber mulch.
