Evidence: The playground had an artificial grass covering that had large wrinkles throughout the play area posing a trip hazard. Near the doorway leading into the school there was a ? inch gap between the artificial grass and the border of the playground, small green pellets were observed in the gap and in the corner of the door leading into the school.
