Evidence: The record of staff #4, employed 01/02/2023, contained a completed national fingerprint search results that was dated 01/03/2022 exceeding the date of employment. All fingerprint search results must be obtained prior to the first day of employment.

Evidence: There was no carbon monoxide detector at the center. The director confirmed a carbon monoxide detector had not been installed.
