In the record was a note from the parent stating it was ok to move the child to the older classroom. The record was missing a written assessment by the program director and program leader. 2. Child # 2 date of birth was 10/08/2019 and on 11/10/2021 the child was moved to the 4 year old classroom. There was no documentation in the child's record that gave permission for the child to be moved to an older grouping of children. The parent was aware of the move to the older class to help the child in a behavioral situation.
