    8VAC20-780-340-A · Based on the review of video footage, the center failed to ensure that when staff are supervising children, they shall always ensure their care, protection and guidance.

    Evidence: Video footage shows Staff 1 sitting in a chair in front of 3 children that were seated in highchairs. Child 1 (7 months old) can be seen falling forward out of the highchair, striking their head on a small refrigerator, landing on their face and shoulder on the floor, and coming to rest on their face/stomach; Child 1 immediately began to cry. The tray of the highchair had not been placed on the highchair nor had Child 1 been strapped into the highchair. Child 1 fell approximately 23 inches from the seat of the highchair to the floor. Staff 1 did not ensure the care and protection of Child 1 when she did not place strap the child into the highchair.
