EVIDENCE: Staff Member #1 did not implement the center's established procedures for appropriate oversight and supervision of children. Staff Member #1 allowed Child #1 (4 years old) to leave the classroom unsupervised, to go to the bathroom. The bathroom is located approximately 100 feet from the classroom and approximately 25 feet from the Center's exit door. Staff Member #1 did not have sight nor sound supervision. Child #1 left the day care using the exit door closest to the bathroom. The door was unlocked and equipped with a break-away bar.
