    22VAC40-185-280-B · Based on observation, the center failed to ensure that all hazardous substances were kept in a locked place using a safe locking method that prevented access by children.

    Evidence: 1. The White Pine classroom with 20 children present from two years of age through five years had multiple cleaning supplies and other hazardous substances stored in an unlocked cabinet which was accessible to the children. There were three commercial sanitizing sprays, a spray bottle of soap and water mixture, and an aerosol can of spray paint on the bottom shelf of the cabinet that was over a counter area. The cabinet was approximately five feet high above the counter; however, there was a wooden two step stool located at the cabinet where the supplies were being stored. 2. There were multiple cleaning supplies in bottles/containers located in a hall closet in the Anita building with four classrooms of infants, toddlers, and two year old children. The closet was not locked. 3. There were cleaning supplies in spray bottles/containers located in an unlocked cabinet above the counter at the sink in a classroom with 14 children present ages three through five years of age. The cabinet was approximately five feet high above the counter; however, there was a wooden two step stool located at the cabinet where the supplies were being stored.

    zones had the required amount of resilient surfacing. Evidence: The swings did not have the required fall zone of 12 feet front and back of the swings. The measurable amount was 7.9 feet. According to the attachment VII in the Standards for Licensed Child Day Center the amount of fall zone is double the height of the top of the swings to the ground. Children were observed on the playground using the swings.

The following were unlocked, Clorox wipes, Odor Ban, 2 containers of Mircoban (an aerosol bottle and a spray bottle) , Cascade, Signet Hard Surface Sanitizer, Windex glass cleaner and a clear glass unlabeled container with a chemical. 3. A cabinet above the counter in the Blue Spruce room there were 8 chemicals that were unlocked. The following were unlocked and accessible to children: Clorox wipes, Cascade, Two containers of Signet Hard Surface Sanitizer, Odor Ban, Lysol aerosol, glass cleaner and a soap and water spray bottle.
