8VAC20-780-40-E · The operational responsibilities of the licensee shall include ensuring that the center's activities, services, and facilities are maintained in compliance with these standards, the center's own policies and procedures that are required by these standards, and the terms of the current license issued by the department.
The center's policies and procedures for identifying where attending children are at all times, playground supervision, and for the departure of children state the following: -That each staff will ensure the care, protection, guidance and sight and sound supervision of all children at all times, by accounting for the children in their care on their attendance accountability checklist before transitioning to another location; -Each staff will also record departure information, name-to-face will be conducted periodically to visually verify each child is present; -Visually verify that all children have left the area before moving to the next location, all programs areas and outdoor spaces will be checked; -Children must be signed out by an authorized person at the time of departure and a visual check of the child completed; -Staff will report any attendance discrepancies to the supervisor immediately, and staff assigned to the main pick-up location (in the building) will be responsible for the master center attendance accountability checklist. On 3/30/26, each staff did not account for children and their departure on their own attendance accountability checklist. A child was marked as departed for the day without a visual check of the child departing with an authorized person. Periodic name-to-face checks were not completed. A visual check of the playground area was not completed before leaving to go inside the center, resulting in Child #1, a 5-year-old, being left alone unsupervised on the playground. Though attendance discrepancies were noticed by staff while on the playground, they did not report them to the supervisor. The master center attendance accountability checklist (in the building) was not used to track when children departed for the day.
