    A melamine sponge/foam abrasive cleaner was not kept in the original container and the sponge was not labeled with the contents. The original packaging lists "avoid accidents, do not use on skin or other parts of the body. Using on skin will likely cause abrasions." On 3/4/2026, staff #1 used the melamine sponge/foam abrasive cleaner to remove markers from child #1's (19 months) face, and forehead, resulting in a chemical burn to the child's face.
