8VAC20-780-340-A · When staff are supervising children, they shall always ensure their care and protection.
On 5/27/2026, Staff #1 sent Child #1, age 4, back into an empty classroom to use the bathroom while the teachers and children were on the playground. Staff #1 was positioned in ratio by the outside door and would not have been able to offer immediate assistance to the child. Child #1 was inside the classroom/bathroom for 4 minutes.

On 5/13/2026, Staff #1 did not follow the centers playground safety procedures that includes provisions for active supervision, positioning of staff in strategic locations, and circulating among children. Interviews revealed Staff #1 was sitting at the playground picnic table for over 30 minutes while wearing ear buds and checking their personal cell phone while in ratio with the PreK classroom.

The document showed a ratio of 2 staff with 26-27 children from 9:15-9:30am, a ratio of 2 staff with 27-29 children from 9:32-10:19am, and a ratio of 2 staff with 27 children from 10:20am-1:00pm.
