In the Center?s Playground Policy, procedure #2 lists ?Teachers must always ensure that children stay within sight and sound supervision, as a teacher should be able to see and hear all children from his/her vantage point?. Child #1 was out of sight and sound supervision of staff on 7/30/2024 for approximately two minutes. Child #1 left the playground area and was found in the parking lot by a parent who notified administration. 2. In the Center?s Playground Policy, procedure #3 lists ?Teachers should position themselves on the playground so that they can collectively monitor all zones of the playground?. 3. On the video footage recorded on 7/30/2024, Staff #2 is stationed at the bottom of the playground hill, standing next to the picnic table, observing the children on the playground. There was a tree in front of the picnic table that obstructs the view of the playground equipment where children were observed playing.

Evidence: 1) During interviews, staff reported that on October 18, 2022, the children in the Twos class were not offered their scheduled morning snack. Staff reported that it was picture day, and they did not want the children?s clothes to get dirty. The morning snack was brought to the classroom, but it was not offered to the children. It was reported that the children were offered lunch at approximately 11:00am when management discovered they did not have a morning snack. 2) The center?s Food Policy for Toddlers and Preschoolers states: ?Four Seasons provides lunch and snacks for all toddler and preschool children. Our menus are posted in the parent information center and meet all USDA requirements. See your child?s class schedule for specific meal times.? The Preschool Schedule indicates the morning snack time is between 9:00-9:15am.
