When staff are supervising children, they shall always ensure their care, protection, and guidance. On May 28, 2026, Infant #1 (10 months of age) was placed on the counter next to the sink to wash their hands after a diaper change by Staff A. Staff A then turned to wash their own hands, without holding onto the infant. Infant #1 fell backwards off the counter, falling approximately three feet onto the floor, and landed on their forehead.
