    On 4/22/2025, staff #1tripped in the toddler classroom resulting in staff #1 falling directly onto child #1. Child #1 fell to the ground and hit their head directly on the floor of the classroom. Child #1 was visibly upset and was found to have a raised bump on his head. Staff #1 failed to comfort or render any first aid to child #1 following the incident.
