8VAC20-780-270-A · Based on observation, the center did not ensure areas and equipment of the center, inside and outside, shall be maintained in a clean, safe, and operable condition.
Evidence: 1. The door leading to the playground had a long metal trim piece alongside the length of the door, the metal was not securely fastened so it moved back and forth which could entangle clothing or snag skin. 2. Trash/litter was observed and included plastic bags, chip wrappers, paper towels, and cups. 3. The outside dryer vent did not have a cover. 4. One picket (board) of the wooden fence was broken and laying on the ground with nails protruding from the wood. 5. The gate lock and brackets were broken and hanging from the gate and were unable to be used. 6. The chicken wire running along the bottom of the fence boards of the playground had some sharp edges sticking out in multiple sections of the fence. 7. The metal gutter running from the roof to the ground had a metal piece with sharp corners protruding outwards. 8. A utility steel box located on the playground was sticking above the ground posing a tripping hazard.
