    8VAC20-780-340-A · When staff are supervising children, they shall always ensure their care, protection, and guidance.

    1. On April 10, 2026, Child A (11-year-old) Child B (12-year-old) were left outside unattended while cleaning up from an outdoor activity. It was determined the children were outside two minutes unattended and unsupervised. 2. At approximately 4:01 pm, Staff 1 went inside the building to get a cart for the outside equipment. Staff 1 told Staff 2 they were going inside the building. 3. At approximately 4:03 pm, Staff 2 went inside the building to look for Staff 1 leaving the two children out of sight and sound supervision. 4. Child A notified Director when they return back in the classroom they were left outside alone. 5. Based on review of attendance on April 10, 2026, at the time of the incident there were 26 children and 5 staff.

    8VAC20-780-340-A · Based on staff interviews and documentation, the staff supervising children did not ensure their care, protection, and guidance.

    Evidence: 1. On April 5, 2023, Child A (7-year-old) left the classroom unattended and went to use the men?s public restroom. It was underdetermined how long the Child A was in the bathroom unattended and unsupervised. 2. At approximately 11:00 am, a community member staff member went to use the public restroom when he noticed Child A was unattended and unsupervised in the restroom. 3. The community staff member notified child day center staff that Child A was in the bathroom. 4. Child A was accompanied back to the classroom by the child day center staff. 5. Based on review of attendance on April 5, 2023, at the time of the incident there were 32 children and 9 staff. Licensing was unable to determine how many children were in the classroom at the time of incident. The center did not have documentation of transitions to different locations.
