Evidence: 1) On the dates of inspection, a copy of the fire department inspection report and a copy of the health department inspection report were not available for review. Although a copy of the fire inspection report was submitted with the application, it was not legible so a new copy had been requested. 2) The program director stated that they did not have available these inspection reports.

Child #3 and Child #5 listed a food allergy in the physical examination included in the record.
