8VAC20-780-40-E · Based on staff interviews and a review of center policies, staff failed to follow their own policy and procedure for the arrival and departure of children to the program.

Evidence: 1. On 9/7/2022, Staff #1 said she took attendance for Room #2. She used the attendance sheet to account for the children as they arrived to Room #2. Staff #1 said she marked Child #1 (8 years old) as being in attendance. Staff# 1 said by 4:05 pm all the children had arrived to the after school program. 2. At 5:15 pm, Staff #2 received a call from Child #1?s parent, saying that the child had not gone to SACC and was found home unsupervised. Child #1 did not arrive to the after school SACC program as expected. 3. The ?SACC Safety, Accountability, Supervision Procedures? were reviewed which stated: ? ?Complete after school attendance form as children arrive and account for all children.? ? ?Implement and account for all children using the 4 part accountability system.? ? ?Use passports and checklist to account for how many and which children are in your care at all times.? ? ?Conduct periodic face and name roster checks.? 4. Staff #1 failed to follow the SACC procedures to account for all children as they arrived to the after school program. She did not complete the face and name roster check and the passport- locator board was not used to account for all the children.
