On September 30, 2025, October 1, 2025, and November 18, 2025, Agent #1, Staff #1 and and Staff #2 were contacted about the outstanding request for Agent #1's out of state for criminal history and child abuse and neglect registry. At that time, the center indicated that Agent #1 would no longer serve as an agent of the licensee; however, on February 11, 2026, it was determined that Agent #1 was still serving in the capacity of an agent who is authorized to act on behalf of the licensee. Agent #1?s out of state criminal history and central registry checks were not requested until February 11, 2026, after Agent #1 was contacted about acting as an agent for the licensee in an official capacity.
