8VAC20-780-40-E · Based on observations, review of a child?s record and interview, the center did not follow its policies and procedures to prevent the use of outdated medication.
Evidence: Child #11 was prescribed and had on site an emergency medication, Albuterol, with 2 puffs for asthma each day at noon and 4:00 p.m. The medication administration log for Child #11 documented that the Albuterol had been administered on 7/29/24, 7/30/24, 7/31/24. The Albuterol expired in February of 2024. In an interview on 7/31/24, the Program Director acknowledged that they did not follow policies and procedures to prevent the use of outdated medication.

The record for child 4 with an enrollment date of 4/26/2021, did not contain documentation of an annual update.

Evidence: On April 23, 2023, it was revealed that child #1 (DOB 7/31/2020) was placed in stroller as a form of confinement on two separate occasions: 1. On May 8, 2023, interviews with staff revealed the following: a. Staff #1 and Staff #2 stated that child #1 a 2-year-old was placed in a stroller during nap time on April 23, 2023. b. Staff #1 and staff #2 stated that child #1 was placed in the stroller because he was running around waking up other children that were napping. 2. Based on video footage taken on April 23, 2023, the following was revealed: a. At approximately 5:30pm staff #1 and staff #2 moved their class across the hall to another classroom because they were leaving for the day. At the time of the transition, child #1 was asleep in a stroller. Child #1 was placed in a stroller because he fell asleep on a swing while outside on the playground. b. Staff #4 stated that child #1 was beginning to wake up. Staff #4 then went to remove child #1 from the stroller and place the child on their lap. Staff #4 stated that he was unable to attend to the other children while holding child #1 and therefore placed the child back into the stroller. c. Shortly after placing the child back in the stroller, child #1?s mother entered the classroom at approximately 5:45. Staff # 4 tried to explain to the parent why child #1 was in the stroller.
