On 03/29/2023 the center was notified of the violation and required to provide documentation within 10 days of the notification that an out-of-state child abuse & neglect registry had been requested. On 05/23/2023, a follow-up review of the record of staff #2 (employed 09/13/2022) did not contain documentation of a request for the out-of-state child abuse and neglect registry. This is a repeat violation from 03/29/2023.
