Staff #1 went to intervene and "grabbed and jerked" Child #1 by the hood of their jacket "causing the child to fall backwards and hit their head on the play structure."

On 11/4/2022 Child #1 (Age 2.5 years) "was playing with a friend and pulled the bookshelf down on top of Child #1 hitting the child's head causing a bump and cut on the child's eye" as stated in the accident report.
