On January 29, 2025, a visitor entered the facility and observed 10 children, ages 2-3, during nap time. Staff #1, responsible for supervising the children, was sitting in a chair with her head down and did not acknowledge the visitor. The visitor walked around the room and into an adjacent room with no adult acknowledging his presence. The visitor exited the facility and then entered again to find Staff #1 still sitting in the chair with her head down. Staff #1 did not acknowledge the visitor until he walked up to her and began speaking.
