The following information was missing for two children (child #1 and child #2), who had enrolled approximately 6 months ago. 1.The home address, and telephone number for each parent who has custody 2.Name, address, and telephone number for each custodial parents place of employment or school attendance. 3.The address for at least one person designated by the parent to contact in case of an emergency and the parent cannot be reached

8VAC20-790-630-A · Areas and equipment of the center, inside and outside, shall be maintained in a clean, safe, and operable condition. Unsafe conditions shall include splintered, cracked or otherwise deteriorating wood; chipped or peeling paint; visible cracks, bending or warping, rusting, or breakage of any equipment; head entrapment hazards; protruding nails, bolts, or other components that entangle clothing or skin. In the multipurpose room there was an area on the wall that had peeling drywall and a wooden table that had a clear material that was peeling off of the table. A portion of the fence around the playground was broken and had become detached from the fence post and there were sharp exposed wires.
