Evidence: -Staff #3 lived in Pennsylvania in the past 5 years. -Staff #3 hire date was 2/12/2024. -Staff # 3 record did not obtain criminal record check results for Pennsylvania - Staff #3 criminal record should be obtain completed before reporting to child day center. - Staff #4 lived in Alaska in there past five years. - Staff #4 hire date was 2/5/2024. - Staff #4 record did not obtain criminal record before reporting to the child day care center.

8VAC20-780-450-A · Based on observation and staff interview, the licensee did not ensure cribs, cots, mats and beds used by children other than infants during the designated rest period or during evening and overnight care shall have linens consisting of a top cover and a bottom cover or a one-piece covering which is open on three edges.

Evidence : In the two year old classroom, there were 5 children with no bottom cover and 1 child with no top and bottom cover. In the Pre-K classroom , there were 5 children with no bottom cover. Confirmed and observed with the director during the tour.

8VAC20-780-80-B-2 · Based on staff interview, the licensee did not ensure the center notified department represenativeas soon as practicable, but not to exceed two business days, of any injury to a child that occurs while the child is under the supervision of the center and requires outside medical attention.

Evidence: Incident report and staff confirmed there was incident on February 28th, 2023 and 911 was called after child #1 fell backwards and hit the back of her head. The center did not report the outside medical attention with in two buisness day. During the inspection, on April 18,2023 inspection confirmed center contacted 911 for medical assistance.

Evidence: The head count sheets maintained by the center for the Infant classroom revealed that the center did not meet the required staff-to-children ratio on the following date: January 24, 2023, from 8:42am to 9:57am; two staff with 11 children

Staff #2 hire date 05/13/2022 and Staff #3 hire date 04/25/2022. There was not a out of state criminal check from the states in which the staff #2 and staff #3 had resided in prior to hire date.
