8VAC20-780-450-A · Based on observation and staff interview, the licensee did not ensure cribs, cots, mats and beds used by children other than infants during the designated rest period or during evening and overnight care shall have linens consisting of a top cover and a bottom cover or a one-piece covering which is open on three edges.
Evidence : In the two year old classroom, there were 5 children with no bottom cover and 1 child with no top and bottom cover. In the Pre-K classroom , there were 5 children with no bottom cover. Confirmed and observed with the director during the tour.

8VAC20-780-80-B-2 · Based on staff interview, the licensee did not ensure the center notified department represenativeas soon as practicable, but not to exceed two business days, of any injury to a child that occurs while the child is under the supervision of the center and requires outside medical attention.
Evidence: Incident report and staff confirmed there was incident on February 28th, 2023 and 911 was called after child #1 fell backwards and hit the back of her head. The center did not report the outside medical attention with in two buisness day. During the inspection, on April 18,2023 inspection confirmed center contacted 911 for medical assistance.
