Children shall be allowed second helpings of food listed in the USDA's child and adult care meal patterns. On June 2, the lunch served to children ages 3-5, did not meet the age-appropriate nutritional requirements regarding portion sizes and second helpings were not available when requested by several children. During the month of May 2025, the breakfasts that were offered did not always meet the age-appropriate nutritional requirements and on more than one occasion only one component out of the three required was offered.

On 3-7-2025,.Staff #1roughly handled Child #1, when they grabbed Child #1 and pulled the child towards themself. Then Staff #1 grabbed the face of Child #1 under the chin and around their cheeks. Staff #1 then turned Child #1 around by the upper (left ) shoulder so that they faced away from Staff #1 and then lowered Child #1 to the ground and then took their pants off. Staff #1 grabbed Child #1 by the upper left arm and shoulder and stood them up by pulling their arm and shoulder. Child #1 sustained bruising on their arm as verified by a physician.

8VAC20-780-340-F · Based on staff interview and documentation, the licensee did not ensure children under 10 years of age always shall be with in actual sight and sound supervision of staff.
Evidence: During inspection on July 11, 2023 , Staff #1 stated child was left on the playground. Staff #1 stated she came in throught the playground door while kids were outside playing. Staff #1 was met in the hallway by parent who saw a child on the playground and did not see staff. Staff #1 immediatley went outside and brought the child#1 in the center. Staff #1 said it was less than a minute from the time she left the playground and kids were on the playground. Staff #1 confirmed the child#1 was alone. Documentation provided by the center description of incident states on April 18 parent states that there was a child outside and she didn't see any teachers. Staff #1 went outside and there was a child outside.
