Documentation of the screening shall be submitted at the time of employment and prior to coming into contact with children.The documentation shall have been completed within the last 30 calendar days of the date of employment and be signed by a physician, physician's designee, or an official of the local health department. Evidence : 1- Staff # 2 hire date was 01/22/24, the TB test results were recieved on 1/27/2024. 2- Staff #5 hire date was 03/04/2024 , the TB results were recieved on 3.07/2024. 3-The TB tests were not completed within 30 days prior to employment.
