Evidence: On 7/19/2024, child #1 (age 3) was missing from the facility. The child was found by a person unknown to the child or the staff at approximately 9:20 am and law enforcement was notified by that person to determine where the child belonged. Staff #2 found the child at approximately 10:05 am. The vendor did not notify the department. The department was notified of the incident by local department official on 7/30/2024.

Evidence: On 7/19/2024, approximately 35 children (infants through school age) and 6 staff were outside for waterplay. Child #1 (age 3) left the play area and walked 0.3 mile to include crossing a 2-lane, well-traveled divided road with a speed limit of 35 mph. Staff were unaware the child had left the center. At approximately 9:20 am, Child #1 was discovered by a person unknown to the child or the center?s staff when the child tapped on a window of their home with a grill poker. The collateral reported that the child was holding the grill poker and chewing on a piece of charcoal from an outside grill. Child #1 was brought into the home and law enforcement was notified by the person in the home. At approximately 10:05 am, staff #2 found the child at the collateral?s home after being directed to the home by neighbors. Child #1 was described as wet and dirty with scratches on her legs and arms and twigs and leaves in her hair. Interviews confirm the child did not have the scratches when she arrived that morning.
