The staff member had been employed by the center previously but had been inactive for more than twelve months which requires a new one to be completed.

1. The director confirmed the cart in the kitchen used to transport the children?s meals had not been cleaned from lunch the day before. It had sticky residue and particles of food left on it. 2. There was a roach inside the refrigerator used to store the food for the children?s meals.

Evidence: The licensing inspector measured a depth of approximately 2" of sand at the base of a playground structure. The director confirmed that the resilient surfacing was inadequate and that the owner has been trying to get additional resilient surfacing. Children were playing on the playground equipment at the time of the inspection.

Evidence: 1. In Staff#1's record (date of hire: 09-16-2022), there was no results found for an out of state central registry for the state of NC. 2. Interview with staff confirmed a copy of results was not available for review during the inspection.

Evidence: The following observations were made during the inspection: 1. In the Jelly fish classroom, the diapering area had peeling paint throughout. 2. In the Sea Turtles classroom, the was multiple areas of peeling paint on the wall and window sills accessible to children in care. 3. In the Seahorse's classroom, the drawer on the diapering area was missing a knob and there was a visible protruding screw. 4. The Toddler playground slide was observed with a broken plastic part a the top which was accessible to children. 5. Photographs of all the areas identified were taken by the inspector.

In the Infant and Toddler classrooms, there were visible cracked and peeling paint on the walls and window sills. 2. In the Sea Turtle classroom, there is a visible crack in multiple on window siding with a sharp edge accessible to the children in care. 3. On the little playground area, there was a cracked picnic table and ride on tikes with a missing door. 4. On the large playground area, the wooden border was cracked in multiple areas. 5. In the Twos area, the diapering area drawers has multiple areas of peeling paint and a missing knob with a protruding screw. 6. In the Threes classroom, the top layer of the white cabinet was cracked.
