22VAC40-185-340-F · Based on staff interviews, the facility did not ensure that all children under 10 years of age always shall be within actual sight and sound supervision of staff.
Evidence: 1) At approximately 10:00 a.m. on May 21, 2021, Staff #1 walked Child #1 (age 5) to their classroom. Staff #1 reported she called out the child's name, left the child in the classroom, and walked back to the lobby. When Staff #1 was walking to the lobby, she passed Staff #2 in the hallway. Staff #2 went to the classroom and found that Child #1 was alone and unsupervised. Staff #1 acknowledged she did not check if there was anyone in the classroom when she left Child #1. It is estimated that Child #1 was out of sight and sound supervision of staff for approximately 1 to 2 minutes.

During interview, a member of management confirmed the tuberculosis screening for Staff #5 and Staff #8 were completed more than 30 days prior to employment and a negative tuberculosis screening for Staff #6 could not be located. Documentation of the screening shall be submitted at the time of employment and prior to coming into contact with children. The documentation shall have been completed within the last 30 calendar days of the date of employment and be signed by a physician, physician's designee, or an official of the local health department.
