    Based on observation, it was determine that inside of classroom was not maintained in a safe manner. Evidence: The Licensing Inspector observed a telephone cord handing in reach of the children in the classroom. The phone and cord was located near the front entrance of the classroom and was long enough to wrap around a child's neck.

    8VAC20-780-270-A · Based on observation, it was determined that the center was not maintained in a safe condition.

    Evidence: The Licensing Inspector (LI) observed a fan sitting on top of an upper cabinet with the cord hanging down the wall and plugged into the outlet. The cord was hanging and in reach of the children in the Preschool III ( 3 years of age) classroom. If the cord was pulled the fan could've fallen on a child. There were 4 children and 1 staff present in this room during the classroom walkthrough.

    8VAC20-780-340-A · Based on interviews, documentation and video footage it was determined that staff did not always ensure the care, protection and guidance of children while in their care. Evidence: On 5/19/2022 at 2:13 p.m. child A (DOB: 08/16/2021) was seated in an infant high chair. Staff A is observed picking up the high chair by the tray at waist level while the child was still seated in the high chair. The tray snapped off of the high chair causing the child and seat to fall to the ground. The chair hit the ground and the child fell back hitting their head on the floor. Staff A did not report the incident to administration until 4:30 p.m.
