    8VAC20-780-500-B · When a child?s clothing or diaper becomes wet or soiled, the child is required to be cleaned and changed immediately.

    On 9/10/25, Child A, 3-years-old, arrived at the center from a public-school bus at approximately 1:00pm. Staff #1 reported checking Child A?s diaper at that time and that the diaper was wet, but not enough to warrant changing it. Staff #1 reported checking Child A?s diaper again at approximately 4:00pm and said it was wet, but did not contain feces, so again did not change the child. When Child A?s parent arrived at the center to pick up the child shortly after 4:30pm, they reportedly noted Child A was walking unusually and later discovered that the child had a diaper rash.

    63.2-1509-A · Based on interview and review of documentation, center staff who had reason to suspect that a child was abused, did not report the matter immediately to the local department. Evidence: On 2/22/24, at approximately 12:20pm, Child A, a three-year-old, had a potty accident. A staff member reported observing Staff #1 grab the face of Child A and use one hand to squeeze the child?s cheeks to force the child to look at them, and then yell in Child A?s face, with their noses touching. The staff member reported that it seemed like an extreme reaction and as if Staff #1 ?fully lost [their] temper.? The staff member did not report the incident to management until approximately 4:05pm on 2/23/24. The center did not report the incident to the local department until approximately 3:30pm on 2/26/24.

    8VAC20-780-500-B · Based on interviews it was determined that not all procedures for diapering were followed to include that children shall be cleaned and changed immediately when their diapers or clothes become wet or soiled. Evidence: 1.) On 05/09/2023 Staff A was assisting Child A (Age 2 years) with toilet training while diapering. Staff A instructed child A to throw their diaper in the trash. The diaper was not disposed of and the wet diaper was still attached around the child's leg and a new diaper was placed on child A. Upon picking up child A from school it was discovered that child A had on two wet diapers. 2.) On 06/09/2023 around 4:00 pm child A was picked up from school with a soiled diaper. Upon review of child A's daily activity report it was discovered that the last diaper change recorded for child A was at 12:45 pm. It was reported by staff B that child A had a diaper change around 3:00 pm, however the diaper change was not documented.
