8VAC20-780-340-F · Based on interviews, it was determined that the center failed to maintain sight and sound supervision of a child under 10 years of age.
Evidence: On 1/26/24 at approximately 11:45 am, child #1 ( DOB 8/15/21) was left alone on the fenced playground while staff #1 and staff #2 and 9- two year old children left the playground and returned to their classroom. Two additional staff inside the center observed child #1 outside alone and returned the child to their two year old classroom. Interviews revealed that the child was left unattended between 1 to 2 minutes.

Based on observation and interviews, it was determined that a contract worker was alone with a child, therefore functioning as an employee of the center, however this person did not have a Fingerprint background check on file, which is required upon hire/functioning as an employee. Evidence: Person A has been acting as an employee since December 2023, however these was no fingerprint background check on file.

8VAC20-780-340-A · Based on interviews and a review of documentation, it was determined that when staff were supervising children, they did not always ensure their care, protection, and guidance.
Evidence: 1. On 8/31/23 at approximately 8:30 am child B, 13 months of age pulled a hot cup of coffee onto their chest. The hot coffee was placed on a lower shelf in the infant room by staff C. The child?s chest turned red and the child cried in discomfort. At the end of the day there remained some red discoloration on the chest that subsided over two days. 2. On 9/7/23 at approximately 12 noon, staff A and B fed child A cheese although the child has a diagnosed food allergy to diary.
