Evidence: 1. During an interview on January 26, 2023, Staff #2 revealed that Child #1 fell from a swing January 20, 2023 resulting in a broken collar bone. 2. Staff #2 stated that Child #1, age 2 years, was lifted onto the swing because the swing was too high for the child to get onto by themselves. 3. Staff #2 stated that Child #1 let go of the swing, fell forward from the swing, landed on their hands on the ground, with their arms straight, which resulted in a broken collar bone. 4. The distance from the bottom of the swing to the ground measured 33?. The swing was observed to be located on a hill and attached to a tree branch.
